Children must be collected promptly at the end of each session.
Football boots cannot be worn in the Sports Hall so please provide
trainers. Food must not be eaten in the Sports Hall at any time.
Water will be provided, but children are welcome to bring their
own drinks if preferred. |

for each child. Complete the form and send it with a cheque made
payable to D.S.S. Ltd to: 22 Ashurst Road, London N12 9AX. Please include a
stamped addressed envelope for confirmation of
the booking, and write your child's name on the back of the
cheque. |
